Social Worker - PRIDE Health : Job Details

Pride Health is hiring a SOCIAL WORKER to support our client's medical facility in NEW YORK and other locations. This is a 14-week assignment (Possibility of extension or conversion) opportunity and a great way to start working with a top-tier healthcare organization!

Job Summary:

The Social Worker will provide essential support and advocacy for individuals and families facing various life challenges, ensuring access to necessary resources and services. This role involves conducting assessments to identify client needs and developing tailored intervention plans to promote well-being and social functioning. The Social Worker will collaborate with community organizations, healthcare providers, and educational institutions to facilitate holistic support for clients. Regular documentation of client progress and outcomes is required to ensure accountability and inform future interventions. The ideal candidate will possess strong communication and interpersonal skills, enabling effective engagement with diverse populations. A commitment to social justice and a deep understanding of community dynamics are essential for success in this role

Key Responsibilities:

Conduct assessments to identify client needs and strengths.

Develop and implement individualized intervention plans.

Provide counseling and support to clients and their families.

Connect clients with community resources and services.

Collaborate with healthcare providers, educators, and community organizations.

Maintain accurate documentation of client progress and outcomes.

Advocate for clients' rights and access to services.

Participate in case conferences and team meetings to discuss client care.

Stay informed about relevant laws, regulations, and best practices in social work.

Qualifications:

• BLS Certification required.
• LMSW license required
• Strong critical thinking, communication, and stress management skills.

• Weekly Pay : $ 2480 to $2680

*Offered pay rate will be based on education, experience, and healthcare credentials.
