Work Shifts 8:00 A.M - 4:00 P.M Job Description Purpose of Position: The major role of the Social Worker/Child Crisis Specialist within the Department of Psychiatry is to function as a member of the PES team responsible for the assessment and determination of the level of services needed by the child/family and following through with referrals for those services. This responsibility may include any, but is not limited to any of the following: Duties and Responsibilities:
- Conducts an initial psychiatric screening/mental status exam to determine the need for and level of intensity of services and formulates a plan of care.
- Functions as a member of the interdisciplinary Child Psychiatry Department Team
- Facilitates transitions from higher intensity ER services to clinic services to assure continuity of care and reduce the need for re-hospitalization or future ER visits
- May facility coordination of care by seeing patients identified by Peds ED in the ED area to arrange for follow-up care either in Child Crisis, Child Psych, or in the Community
- Works with Family to identify available resources and supports
- Educates family and patient about the child's illness/behavioral challenges makes treatment recommendations and facilitates linkages to a broad range of mental health, community, and school-based services
- Provides follow-up with treatment providers and the patient's family with regard to adherence to the Team's recommendations
- Works with child and family on initial contact to engage them while providing initial therapeutic interventions to motivate and encourage engagement in aftercare/ongoing treatment
- Will assist in data collection and the maintaining and reporting of information on the patients/families seen and outreach, contacts, and referrals made.
Educational/Training Responsibilities:
- Coordinate with Schools and/or CBO based Mental Health Providers, Foster care agencies, ACS and Child Mobile crisis teams
- Offer to educate community schools on how to make referrals to the Child Crisis Team directly, avoiding the emergency Department process when possible
- Offer training and information to other Bronx-based CBOs to facilitate referrals to and from outside providers
