The social worker works closely with the patient/caregiver's and other team members to assist the patient in identifying and utilizing appropriate community services. The social worker will act as a liaison between the family, staff, community institutions and agencies to meet the needs of the patients. The social worker coordinates the bereavement program for Hospice and provides grief and loss support to Hospice patients and their caregivers.QualificationsWhat is required...
- Completion of an accredited Social Work program with a Master's degree
- 1-2 years' experience in a healthcare setting, preferably with home health and Medicare experience;
- Current BLS for Healthcare Provider card (maintenance required);
- Valid Washington State Driver's License;
- Reliable transportation & valid automobile insurance.

- Current license from Washington State Department of Health as a licensed advanced social worker, licensed independent social worker or licensed social work associate

Additional InformationWage: Wage range currently starts at $30.89/hour and goes up to $46.82/hour. Actual wage will be based on years' of applicable experience.
