Overview:

Responsible for consultation and direction of social services in the acute and skilled nursing facility. Provides orientation and education to the social service and nursing staff. Develops, organizes and implements programs, policies and procedures, conducts case work services and counseling and recommends functions necessary to ensure overall operational viability. Maintains performance improvement activities within the department and participates in CQI activities.

Responsibilities:
  • Assures that social service/discharge planning activities are integrated and coordinated within the hospital system by effective communication at all levels.
  • Develops specific, measurable, realistic plans and objectives, which enhance social service delivery.
  • Ability to perform a psychosocial assessment and reassessment on all patients. This includes pediatric, adolescent, geriatric and the general patient population.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate.
  • Demonstrates knowledge of clinical social work and discharge planning.
  • Treats patients and their families with respect and dignity. Identifies and addresses psychosocial needs of patients and family.
  • Manages and operates equipment safely and correctly.
  • Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.
  • Communicates appropriately and clearly to physicians, staff, Nurse Executive and administrative team.
  • Coordinates and directs social services to ensure patients needs are met and hospital policy is followed.
  • Provides appropriate resources and support to social services designee, both in the acute and skilled facilities. Demonstrates an ability to be flexible, organized and function under stressful situations.
  • Maintains a good working relationship both within the department and with other departments.
  • Consults other departments as appropriate to collaborate in patient care and performance improvement activities.
  • Maintains performance improvement activities for department.
  • Documentation meets current standards and policies.
  • Provides education to staff on performance improvement. Ability to plan and organize orientation and in-service training for staff members. Participates in guidance and educational programs.
  • Performs consultative activities including recommendations for changes in service delivery.
Qualifications:

Education:

  • Master's Degree in Social Work form an accredited school, required.

Experience:

  • Minimum three (3) years of previous experience in all aspects of social services and discharge planning.

Knowledge and Skills:

  • Ability to communicate effectively in English both verbally and in writing.
  • Basic computer knowledge.

Licenses, Certifications:

  • New York State Social Worker License (LMSW) required.

Physical Requirements:

  • Involves standing, walking, sitting, talking, hearing and bending.
  • May include some repetitive motions.
