Social Worker, Orange and Rockland County - Social Work PRN : Job Details

Description Social Worker needed for 1 position in Orange County and 1 position in Rockland County (2 separate positions, 1 for each county) - in-person and home visits required. Responsibilities: •Contributes to patient and family counseling and education •Participates in ongoing interdisciplinary comprehensive assessment •Assesses patient/family psychosocial response to the terminal illness and evaluates coping skills •Assists patients, family, and agency staff in utilizing community resources •Assists team members in understanding the significant social and emotional factors related to the care of the terminally ill patient and family •Coordinates with team members to plan and implement the hospice plan of care •Social Worker will participate in interdisciplinary team meetings •Social Worker maintains accurate, thorough and professional written records, and charts relevant information in a timely fashion, following UH documentation standards •Participates in and is actively involved with the bereavement portion of the program as appropriate •Participates in volunteer and staff education programs •Develops and implements the discharge plan in conjunction with the IDS •Participates in QAPI program and hospice sponsored in-service training •Refers high risk patients •Develops/maintains community resource file •Incorporates organization's commitment to customer service and continuous quality improvement into daily work habits. This includes promoting positive co-worker and teamwork relationships and supporting the goals and mission as set forth by the agency •Accepts additional responsibilities as assigned Salary Range: $31/hr. - 38/hr. (approx. $56,000-$79,000) and is commensurate with education, applicable experience, licensing and certifications. Position Requirements •MSW from a graduate school of social work is required. Must be licensed by the State of New York •LMSW required, LCSW a plus •Must have at least two years of supervised experience in the health field or two years of experience in family therapy/counseling •Social Worker in this role must receive certification in hospice and palliative care within three years of hire date and maintain certification during employment •Ability to relate to a diverse population and workforce in a personable and effective manner •Ability to relate to the terminally ill and their families; bereavement experience preferred •Must be proficient in MS Office Suite, EMRs and able to learn new software systems •Must have daily access to a vehicle, a valid driver's license/acceptable driving record and insurance coverage for daily travel to patient locations This position is currently accepting applications.
