Social Worker (PACE) - Baptist Health : Job Details

Shift: DayWorking Hours: 0800-1700Summary:Responsible for the direct social work case management services to PACE participants. Provides social service support to include participant screening and assessments, case management, counseling and referral.Other information:•Master's degree required•LMSW or LCSW required•Two years experience in healthcare preferred•Experience working with the frail and elderly population preferred•Excellent customer service skills•Excellent interpersonal communication skillsThis job will be authorized 80.00 hours bi-weekly.
