Job Type Part-timeDescriptionProvides psychosocial support and counseling to patients, families and caregivers and identifies and coordinates community resource needs. Responsibilities:
Ensures that patient/family psychosocial issues are addressed and that community resource needs are identified for each patient/family.Performs and completes patient/family initial and on-going bio-psychosocial assessments consistent with YoloCares policies and procedures; ensure initial assessments are completed within five days of social work start date.Contributes to the IDT in establishing and maintaining a Plan of Care for each patient/family.Provides direct clinical services to patients/families, as indicated by the initial and ongoing bio-psychosocial assessment and as directed by the Plan of Care, documents accordingly.Provides clinical consultation to the IDT members, health care staff and collaborates with community referral agencies/persons, observing confidentiality regulations and policies. Identifies and coordinates community referral required to support or enhance the treatment of patient/family. Participates in weekly IDT meetings.Works with Bereavement Services Manager to provide bereavement and/or anticipatory grief counseling/therapy to patient/family.Provides admission assessments social work documentation for YoloCares Palliative Care Program.Provides visitation/phone calls to Palliative Care program as directed by plan of care or by RN/Program Manager.Note: The above statements are intended to be a representative summary of the major duties and responsibilities performed by the incumbent of this job. Consistently meet all standards of conduct delineated in the Yolo Hospice Integrity Program: Standards of Conduct: A Guide to Compliance and Ethics.RequirementsQualifications:Must have a Bachelor of Social Work Degree. Knowledge of group process and ability to facilitate an interdisciplinary approach to hospice and palliative care.Highly skilled in both written and verbal communication.Knowledge of community resources. Knowledge of child and elder abuse reporting requirements and other significant legislation and laws regulating hospice practice. Knowledge and understanding of individual, child and family development, human behavior and communication, the influence of culture and values in Social Work practice and medicine. Electronic medical records experience is preferred.Ability to successfully meet agency background screening requirementsJob Specifications:Able to hear, speak, read, write, and/or type well enough to perform required job duties.Ability and willingness to maintain and drive a personal vehicle. A valid California driver's license and state-required automobile insurance are required. Schedule: 20-24 hours per weekSalary Description $25 - $30/ Hourly
