The Social Worker under the direction of the Program Coordinator is responsible for initial and ongoing needs assessment, individual service plans, referral and advocacy for individuals who reside in permanent supportive housing for women and women with children who have experienced chronic homelessness, mental illness or substance use disorder or a physical disability. The Social Worker's caseload may include both single women and women with children. The position will require occasional weekend and evening work hours. The Social Worker will offer specialized services focused on addressing the complex issues of trauma, mental illness, addiction, family stability, positive parenting and family interaction designed to support housing stability, family stabilization, independent living, financial security. PRINCIPAL RESPONSIBILITIES:

  • Conduct intake interviews, collect all required documentation, review general program information and rules with all
new tenants.
  • Conduct initial and ongoing needs assessment and develop individual, person centered service plans collaboratively
with resident that support goal achievement. Initial intake must be completed within 72 hours of tenant move in.
  • Meet regularly with tenants to develop quarterly and annual individual service plan as required, meet with tenant
regularly to discuss goals. Obtain all required signatures on service plans as required.
  • Maintain accurate and up-to-date documentation of services, needs and progress for all household members;
maintain records, files and correspondence in written and electronic form, including but not limited to service plans, AWARDS case notes, etc. Document all tenant interactions in AWARDS within 48 hours of interaction.
  • Provide general case management functions, including assistance in obtaining any identification, documentation, or
entitlements needed for access to community-based services.
  • Maintain Housing goal of zero rental arrears for each family and/or individual.
  • Conduct psychosocial assessments and make referrals for services as appropriate.
  • Provide counseling sessions and support services as needed.
  • Facilitate group sessions and develop curriculum for support groups and special supportive projects such as a harm
reduction, recovery groups, meditation and mindfulness, etc... as required.
  • Provide ongoing emergency crisis and conflict management to support tenants in maintaining physical, emotional,
and social safety.
  • Provide referrals to community-based agencies and resources as appropriate. Escort tenants to appointments as
needed.
  • Work with individual and/or the entire family to improve family functioning
  • Participate in and co-facilitate clinical case conferences as part of a team.
Conduct monthly home visits for entire caseload
  • Participate in individual supervisory meetings and case reviews with immediate supervisor as required.
Attend meetings and staff training as required.
  • Perform program objectives in accordance with all Supportive Housing protocols according to HousingPlus and
funder regulations and policies and procedures.
  • Performs other duties as assigned.
Requirements Master's Degree in Social Work or other related degree, LMSW/LCSW preferred.
  • Two (2) years of case work experience, preferably working with women with histories of homelessness, mental illness
substance use disorder or physical disability and criminal justice involvement.
  • Experience with trauma informed and responsive service delivery.
  • Excellent verbal and written communication skills and the ability to work independently.
  • Excellent interpersonal and organizational skills.
  • Competent technology & management skills, including proficiency with using Microsoft Office; and client database
management systems.
  • Must be able to walk distances of .25 miles, walk over uneven ground, and climb a minimum of 3 flights of stairs.
  • Evening Hours Required
