Desert View provides safe and competent nursing care for patients in accordance with facility policies, standards, and philosophy. We provide professional leadership and support to the healthcare team. The concepts of Patient Centered Care will provide the foundation for all nursing care. Demonstrates Service Excellence standards at all times.The Social Worker will provide comprehensive psychosocial services to patients and their families, assisting them in coping with the challenges associated with acute care hospitalization. This role involves conducting assessments, offering counseling, facilitating access to community resources, and collaborating with the healthcare team to develop and implement individualized care plans.Key Responsibilities:Psychosocial Assessments: Conduct thorough assessments to identify patients' emotional, social, and environmental strengths and challenges.Counseling and Support: Provide individual and family counseling to help patients and their families manage the psychological and social impacts of illness and hospitalization.Discharge Planning: Collaborate with the healthcare team to develop and implement discharge plans that ensure continuity of care and optimal patient outcomes.Resource Coordination: Connect patients and families with appropriate community resources, such as support groups, financial assistance programs, and home care services.Crisis Intervention: Offer immediate support and intervention in crisis situations, working to stabilize and support patients and families during acute episodes.Documentation: Maintain accurate and timely records of patient interactions, assessments, care plans, and interventions in accordance with hospital policies and regulatory requirements.Team Collaboration: Actively participate in multidisciplinary team meetings, contributing to the development of comprehensive patient care plans.Qualifications:Education: Bachelor's or Master's degree in Social Work from an accredited institution.Licensure: Current and valid State of Nevada license as a Social Worker.Experience: At least one year of professional social work experience in a hospital or healthcare setting is preferred.INDEED.COMSkills: Strong assessment, counseling, and crisis intervention skills; excellent communication and interpersonal abilities; proficiency in developing and implementing discharge plans; knowledge of community resources; ability to work collaboratively within a multidisciplinary team.
