Social Worker - Substance Abuse Specialist 237660 - Medix™ : Job Details

Responsibilities

• Work with ACT Team members to provide evidence-based mental health and substance use interventions to program participants.
• Conduct home and institutional visits to engage clients in substance use recovery. (Field work required)
• Administer screening tools to evaluate substance use and co-occurring mental health needs.
• Design, implement, monitor, and document treatment plans with a focus on substance use recovery.
• Provide education and referrals to clients and their support networks regarding substance use resources.
• Maintain detailed case records and submit reports within required timeframes using electronic systems (EHR, Google Docs, Microsoft Excel, etc.).
• Establish partnerships with external agencies, programs, and services that specialize in substance use recovery.
• Participate in clinical supervision, interdisciplinary team meetings, and staff development activities.
• Support program planning and represent the agency at community events as needed.
• Ensure compliance with all regulatory and programmatic standards.
• Rotate on-call responsibilities with team members to provide 24/7 crisis response.
• Perform other tasks as assigned.

Qualifications

• Master's degree in Social Work, Mental Health Counseling, or a related field required.
• CASAC-T certification or significant experience working with substance use issues is required.
• New York State professional license (LCSW, LMSW, LMHC, or equivalent) preferred.
• Two years of experience in a mental health or substance use treatment setting preferred.
• Excellent communication, problem-solving, and organizational skills.
• Ability to work in a culturally sensitive and inclusive manner.
• Basic computer proficiency; familiarity with electronic client record systems preferred.
