Your Specific Responsibilities:

This position i s a Peer Care & Treatment Navigator for Area 3/13.

This i s a Career Service position that works direct l y w i th new and existing c l i ents eligible for Ryan White Services and support across the 15 counties in Area 3/13.

This posi tion:

+ Provides Peer Services for Area 3/13 HIV Clients.

+ Call clients with appointment reminders for eligibility, ADAP, case management, doctor, and lab appointments. Meet with new Clients as they enter or re-enter the Area 3/13 County Health Department's specialty care clinics. Follow-up with Clients to overcome barriers to care and remain in care. Work on linkage to care program. Assist Clients with adherence. Accompany/transport Clients to appointments if available and requested. Complete forms and documentation accurately and as instructed. Communicate with CHD clinic staff, Ryan White eligibility, and case management staff as needed and directed. Attend meetings and clinics as directed.

+ When clinic schedule changes, will call and reschedule appointments. Assures that labs are present electronically in preparation of clinic visits. Performs necessary check-in functions, including financial eligibility status screening, verifying current insurance status, and updating demographics, phone numbers and address and ensuring proof of all positives are in the ELO of all CD clients. If needed, will scan lab results from outside sources and schedule follow-up appointments with providers as needed. Checks with the phone triage daily for any incoming faxes. Responsible for canceling appointments due to termination of insurance coverage or ineligibility. Communicates all scheduling changes with supervisor.

At all times maintains confidential information such as social security numbers, medical information and other confidential data in a secure manner. Respects the confidentiality of related issues by not discussing with unauthorized staff. Per DOH Information Security and Privacy Policy 7-Confidential Information, staff have the authority to take confidential information into the field to perform job responsibilities. Staff must adhere to Field Policy for Confidential information. Ensures discussions held between staff cannot be overheard by unauthorized staff. Reports suspected breaches of security to supervisor immediately

Required Knowledge, Skills, and Abilities:

Knowledge, skills, and abilities, including utilization of equipment, required for the position:

+ Knowledge of Microsoft Systems

+ Experience working in Electronic Medical Records

+ Strong Organization, Communication and Customer Service skills

Qualifications:

Minimum - Must have experience living with HIV and/or personal experience with Pre-exposure Prophylaxis for HIV.

Must have knowledge, skill, and ability to use Microsoft Office Programs.

Must have valid driver's license and access to reliable transportation.
