Our flagship transitional work program Ready, Willing & Able integrates paid work, occupational training, and holistic supportive services to facilitate an individual's return to mainstream society.The Doe Fund has an annual operating budget of $65 million and a full-time staff of 400+.We are seeking a Case Manager/Substance Abuse Counselor for The Doe Fund's newest permanent supportive housing program for homeless men and women living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. Forty-one units will be allocated for residents referred by the New York City HIV/AIDS Services Administration (HASA) and twenty-seven apartments will be for adults referred by the New York City Housing Authority (NYCHA). The Case Manager/Substance Abuse Counselor is responsible for providing supportive case management services to the HASA-funded residents. The successful candidate will have strong clinical and communication skills along with an understanding of working with the HIV/AIDS population.RESPONSIBILITIES:Maintain a caseload and provide regular case management services to these individualsMaintain resident records in compliance with agency and funding standardsConduct and review assessmentsDevelop and implement individual Service Plans and assist residents with achieving their goals geared towards self sufficiencyMonitor and ensure residentsare linked to healthcare providersProvide counseling and crisis intervention as necessaryAssist residents in securing and maintaining benefitsMaintain a liaison with community services including hospitals, substance abuse treatment programs, HRA and mental health programsFacilitate groups and activities that promote health and independent living skills developmentMonitor and assist residents with rent paymentsConduct regular unit inspectionsCompile resources on substance abuseEducate residents and staff about approaches to managing addictionOversee recovery readiness, relapse prevention, and recovery oriented servicesOrganize sober recreational activitiesAttend ongoing trainings to enhance the services provided to the residentsAssist with completion of HASA's weekly and monthly reports as well as any other relevant reportsOther duties as assignedQUALIFICATIONS:Bachelor's Degree in Social Work or relevant fieldCredentialed Alcohol and Substance AbuseCounselor(CASAC) certification or a License in Clinical Social Work(LCSW) is strongly preferredIdeal candidate must have at least one year of experience working with people who are homeless, living with HIV/AIDS, have substance abuse histories or suffer from mental illnessAt least 3 years of experience in substance abuse counselingKnowledge of benefits in the New York City Metro area, HIV/AIDS, psychiatric diagnoses, substance abuse, and mental health issuesSolid computer skills - internet, Microsoft Office, and charting databaseStrong organizational, writing, and communication skillsSpanish speaking a plus, but not necessaryInterested?Submit your application for review.
