Washington State Department of Retirement Status Form; 6. Immunization History (for persons born January 1, 1957 or after) - Completion of District Form to be placed on file with the District Nurse; 7. Online mandatory trainings per Valley School District policy; 8. Hepatitis B Training (if required); 9. Social Security Card and valid Driver's License (for identification upon hiring), to be copied by Personnel Office. BY SELECTING YES , YOU AGREE TO PROVIDE /COMPLETE THESE REQUIRED ITEMS.

* I authorize Valley School District #070 to make an investigation of my personal, educational, vocational, or employment history.
