*$3,000 Sign-on Bonus Now Available**Free health insurance for MHA full-time employees*OverviewMHA in Ulster County, Inc. has a full-time position (37.5 hours per week) available in our Residential Program. The Supportive Housing Case Coordinator maintains a caseload and works with clients, who reside in their own apartments and receive rental assistance, to ensure long-term housing sustainability. Case Coordination is specifically focused on housing maintenance and stability: paying rent, interacting positively with landlords and neighbors, managing behaviors that may accompany mental illness or substance use so that they don't interfere with success in housing, developing crisis plans, connecting with appropriate community resources, and supporting clients' individual housing goals.Available schedule is Monday-Friday 9am-5pm.The Supportive Housing Case Coordinator has a starting hourly pay rate of $19.20.ResponsibilitiesWork with the Residential Director and Site Supervisor to meet program/department goals and objectives, both independently and as a team, and ensure and maintain the confidentiality of all agency program participants and department information.Work with each client to develop and progress on goal plans according to individual goals and objectives.Maintain contact with all housing clients on a scheduled basis, appropriate to their particular status and needs.Provide referrals to services such as primary and specialist health care, mental health and substance abuse treatment, benefit entitlements, nutritional support, legal services, and all other available community services and support client in navigating this process.Document all client interaction and maintain accurate paperwork/reports.QualificationsInterested candidates must have a High School Diploma and a minimum of one (1) year experience in Human Services.This individual must have excellent writing, computer, organizational, and interpersonal skills and must be able to prioritize tasks and work independently.Must have a clean, valid NYS driver's license with 3 years driving experience.MHA in Ulster is proud to offer the following benefits to our full-time employees:Paid Vacation / Sick / and Flex DaysMedical / Dental / Vision / Life InsuranceRetirement 403(b) PlanProfit SharingSupplemental Benefits such as TelemedicineAnd More!
