About the Job We are looking for a Supportive Housing Case Manager to work at our Bay Apartments transitional housing program in Coos Bay, Oregon. The Supportive Housing Case Manager will provide recovery support services to clients with severe and persistent mental illness referred by Coos County and collaboratively works with a multi-disciplinary team in identifying client needs and determining appropriate community resources to resolve identified needs. The Supportive Housing Case Manager will provide skills training to promote successful community living for individuals suffering from mental illness who have high hospital utilization, minimal successful independent living experiences, lack of appropriate health care, cultural or language barriers, behavioral issues, and other characteristics that serve as barriers to independent living. Vocational Training ProgramFor applicants starting as a QMHA-R, ColumbiaCare offers a Vocational Training Program to accelerate the training, on-the-job experiential learning, and testing preparation for qualified employees to achieve the 1000 supervised hours required to complete their certification as a QMHA-I. Through this program, this certification can be achieved in as little as 6 months. This is an optional program. Work Schedule: Monday through Friday, 8:00am 5:00pm (Full Time, Day) What You ll Make$22.00 - $25.20 per hour DOE/Credentials.Additional 5% Language Differential offered for Bilingual or Multilingual candidates (Spanish/English desired). About the ProgramBay Apartments (est .2007) is a 10-unit studio apartment project in Coos Bay. These 10 studio apartments are rental housing units for transitional housing serving Coos County mentally ill adults for up to 18 months. They are designed to help people get back into the regular commercial housing market by providing a transition from homelessness to independent living. ColumbiaCare Services is a qualifying employer for the Public Service Loan Forgiveness (PSLF) Program. QMHA employees are also eligible to apply for the Oregon Behavioral Health Loan Repayment Program. What You ll NeedMust be eligible for Qualified Mental Health Associate (QMHA) registration. Eligibility requires 1 out of 3 Criteria below:Bachelor s degree in Behavioral Science: Psychology, Counseling, Social Work, Human Development, or related field; OR minimum three (3) years of verifiable behavioral health experience working with individuals with behavioral health needs in a supervised setting; OR equivalent combination of behavioral science education in addition to satisfactory supplemental behavioral health experience and training If not already registered as a QMHA through the Mental Health & Addiction Certification Board of Oregon (MHACBO), we will assist you in registering within 30 days of hire. Vocational Training Program: Employees in the optional Vocational Training Program will have additional training expectations to meet the requirements for QMHA testing. Driving is an essential function of this position. Successful applicants MUST also have a valid Oregon driver s license, maintain a safe driving record, and have the ability to pass a DHS criminal background check. Physical Requirements: This position requires the ability to frequently sit, talk, listen, and use hands and fingers. It may require the ability to occasionally walk, climb stairs and ladders, bend, stoop, squat/kneel, and perform other physical tasks as applicable; as well as lift, carry, push, and pull up to 30 pounds.
