Job Description :

We are seeking a full-time Community Support Team Lead to provide community-based therapy and case management to adults. The therapist will conduct assessments and provide community-based therapy and case management to adults, including initial assessments for clients with serious and persistent mental health issues and connecting them with a higher level of care such as ACT services.

Responsibilities:

  • Provide assistance and support to mental health consumers in crisis situations.
  • Conduct service coordination activities, with or on behalf of a client.
  • Engage in psycho-educational and support activities for clients and their families and/or natural supports as appropriate.
  • Support rehabilitative and independent living skills.
  • Monitor and manage symptoms.
  • Assess and reassess the client's need for services.
  • Inform clients about benefits, community resources, and services.
  • Assist clients in accessing benefits and services.
  • Arrange for clients to receive benefits and services.
  • Monitor the provision of services.
  • Qualifications:

  • At least one year of experience with the population served.
  • Must have a Provisional or Full license in NC (i.e LCSW/A, LCMHC/A).
  • Location: Smithfield, NC

    This is an off-site job aimed at providing these services in the community to enhance the consumer's ability to function and achieve their goals.
