Job Description :

The Team Leader is responsible for the coordination of and the delivery of high quality services as determined by the individual members' assessments and program plans.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Ensure ongoing compliance with appropriate local, state, and federal regulatory requirements.
  • Assume accountability for the implementation of approved policies and procedures.
  • Direct and supervise scheduling of employees.
  • Provide management and communicate with team members.
  • Actively promote staff participation, teambuilding, and involvement in the day-to-day operations and decision making.
  • Perform and direct member care including assistance/prompting.
  • Ensure a safe and sanitary environment is provided.
  • Initiate needed repairs and maintenance of site house and grounds.
  • Assist with training employees, conduct counseling interviews and initiate disciplinary action.
  • Prepare written employee performance evaluations.
  • Monitor and maintain records reflecting member's physical and mental condition.
  • Coordinate and complete the admit procedure.
  • Coordinate, complete, and monitor discharge procedure.
  • Inform the Administrator of any problems or circumstances.
  • Be on-call 24 hours a day and 7 days per week to address emergencies immediately.
  • Assist with cleaning with the members and around the home, helping with laundry, getting members to appointments, prompting members with activities of daily living, cooking, passing medications, teaching members about their medications, helping with activities, and ensuring members' safety.

    • TYPICAL QUALIFICATIONS/EDUCATION/SKILLS:

    • Must be at least 18 years old.
    • High School Diploma or equivalent is preferred.
    • Clean driving record.
    • Mandatory Adult Abuse Reporter Certificate or obtain within 60 days of employment.
    • Chauffeur's license or obtain immediately after hire.
    • Must have a desire to work with persons with disabilities.
    • Pre-employment background check, drug screen, and physical required.

    Job Type: Full-time

    Pay: $13.50 - $14.50 per hour

    Expected hours: 30 – 45 per week
