Southcoast Health is an Equal Opportunity Employer.Compensation: Pay rate will be determined based on level of experience.ResponsibilitiesUnder direction of the Director of Case Management or designee, perform diversified duties including but not limited to communicating with Skilled Nursing Facilities Owners for the overall strategy Southcoast Hospitals. Coordinating all data and managing relationships with preferred SNFs and the SNFs bed placement Team. Leading all meetings with Patient Financial Services and Case Management documenting the outcomes of all productivity each month to meet KPIs. Lead the relationship with the SNF and communicate with their owners or Directors on monthly data and patient KPI for Quality reporting.Qualifications

• Current licensure by Board of Registration in Nursing RN is required.
• One to three years nursing experience is required.
• At least two years of experience in an home health or inpatient case management is required.
• Preferable in an administrative role Must possess and demonstrate highly effective interpersonal, customer service and communication skills.
• Requires proficient computer skills, including experience with various software, database, and electronic records programs.
• Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.

Pay Range USD $46.11 - USD $82.21 /Hr.
