Job Description :

Eliot's Homeless Division is seeking an individual with strong engagement, support planning/implementation, and organizational skills to take on the role of Team Lead of CSPECH Programming. The ideal candidate will have knowledge/experience in supporting homeless adults with mental illness and/or substance abuse disorders, as well as knowledge of public entitlements (EAEDC, Social Security, Food stamps, etc.) and other resources.

Eliot's Homeless Services provides services to those with serious mental health illnesses or with co-occurring substance use disorders, who are homeless or at risk of becoming homeless. Our clinicians provide support services directly to the homeless in emergency shelters or on the streets of Massachusetts.

Qualifications:

  • Must have a Bachelor's degree in psychology, counseling, social work, nursing or related field. Master's degree in healing arts preferred but not required.
  • At least 2 years direct experience with underserved populations, including 1 year managerial experience.
  • Must have strong assessment, treatment planning/implementation, and organizational skills.
  • Ability to provide clinical and administrative guidance to clinical/paraprofessional staff.
  • Maintain knowledge of and methods to identify and access community resources - especially housing systems (COC/Balance of State/Permanent, Project Based, Transitional Housing).
  • Must have a valid driver's license and reliable transportation.

Responsibilities:

  • Support the Director of CSPECH Services with clinical and administrative oversight of CSP-HI Services including program growth/expansion.
  • Provide onboarding support and training to new hires related to policies and procedures.
  • Provide real-time guidance, consultation, and consistent weekly supervision to assigned Housing First Coordinators and Recovery Coaches – including side-by-side field learning.
  • Ensure CSP-HI Performance Specifications established by MassHealth are met in collaboration with the Director of CSPECH and Eliot's Quality Assurance Team.
  • Provide comprehensive assessment of and triage incoming CSP-HI referrals; assess for program eligibility; liaise with internal/external partners regarding service needs and risk.
  • Coordinate efforts with ACO partners and other community service providers to maximize resources in collaboration with the respective client.
  • Maintain a thorough, current knowledge of local public and private resources including entitlement/benefit programs, housing, residential, and other community services.
  • Maintain frequent contact with property managers and landlords (including Boston Housing Authority) to determine appropriate services for residents of the program, and to ensure a positive relationship with housing providers.
  • Carry an abbreviated caseload and evaluate clients' capacity to make independent living decisions, including risk assessments for clients around safety and tenancy preservation.
  • Develop and modify individual client service plans to assess ongoing needs for clinical, supportive services, and housing needs.
  • Support and collaborate with the CSP-HI Operations Manager with logistical and/or systemic tasks as needed and applicable.
  • Hold weekly group supervision meetings with supervisees to provide support and guidance. Flag high-risk situations and escalate to the Director of CSPECH as needed and applicable.
  • Comply with all statutes and regulations relevant to the maintenance of clinical standards in the Commonwealth, e.g., mandated reporting and duty to warn.
  • Other duties as assigned.

6% additional pay increase for bilingual in a needed language based on program needs.
