Transfer Center is the logistical hub of patient movement into and within the SLHS. The THS role is a dual role within the SLHS Transfer Center (TC). In the Patient Placement (PPO) role the THS partners with internal stakeholders to place patients quickly to the appropriate care area at our four main campuses. The THS utilizes the bed board in Epic to place patients in their assigned beds and communicates with stakeholders typically via text. When taking TC calls, the THS connects referring and receiving providers to move patients into and within the SLHS efficiently and safely. There is no patient interaction in the TC; however, this role has huge impact on patient outcomes. There are RNs within the TC to help triage and complete complex calls.
