CareStar, Inc. is currently seeking a Transition Coordinator in the Operations Department to assist individuals served with locating housing, coordinating the physical move to a community setting, and are responsible for efficient utilization of program funds. The Transition Coordinator collaborates with long-term care (LTC) facilities, Case Managers and individuals served to initiate services that meet the individual's needs in a cost effective and service efficient manner, with sensitivity given to individuals' personal preferences, rights to self-determination, strengths, needs and barriers.THE POSITION CareStar is looking for motivated individuals who/to:
- Act as point-of-contact to assist the individual to live in his/her home and/or community.
- Assist individuals served with linkages to service providers and community resources as needed.
- Identify barriers to compliance with individual's healthcare regime, appointments and screenings.
- Work with individuals to create solutions to identified barriers and empower individuals to manage physician, and/or other key medical professionals, contacts, appointments and/or screenings.
- Communicate and collaborate with the individual's healthcare team, including, but not limited to, the facility Social Worker and Waiver Case Manager.
- At least 18 years of age or older.
- High School Diploma or higher education level.
- Minimum of 12 months of experience delivering services in a healthcare, behavioral health or social services setting.
- Experience advocating on behalf of individuals with chronic illness, mental health and/or substance use disorders, and physical disabilities.

- Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
