Job Description The Salvation Army Midland Division Veteran's Residence is looking for a Transition in Place Case Manager to supervisor and coordinates the engagement in the assist veterans with transitional support. This is a full-time, 40 hours per week, $44,000.00 per year. 2935 Locust, Saint Louis, Missouri EssentialFunctions
  • Professionally complete case management functions such as intakes, assessments, service/care plans, progress notes, case coordination notes, incident reports, notices, discharge summary, and post discharge contacts within required timelines.
  • Complete all documentation, reports, etc. using professional language for the field of practice.
  • Complete home visits with Housing Specialist to ensure a safe environment.
  • Complete and provide daily, weekly, monthly, quarterly, and bi-annual outcome measurements and report progress to supervisor.
  • Organize and update client files utilizing professionally established practices for file maintenance.
  • Conduct timely and detailed case management interactions, as required, and follow up with case coordination with Housing Specialist to facilitate the process of clients meeting their goals.
  • Conduct weekly educational sessions that incorporates life skills, employment skills, community resources for client, etc. as required for housing retention.
  • Participate in Salvation Army Social Service required activities, training, and professional development events.
  • Utilize community resources with Housing Specialist to enhance support for clients.
  • Participate in weekly staff meetings (TIP program and treatment team), weekly/bi-weekly/monthly supervision.
  • Coordinate outreach, admission, and discharge with Program Director, Health Care for Homeless Veteran (HCHV) and Midtown Service and Treatment Center.
  • Provide updates, steps, insight of services to Program Directors (PD) regarding client progress, client off site living and resources needed, to accomplish client goals.
  • Utilize recommended approaches and modules to support the goals of the TIP program.
  • Maintain HMIS (Homeless Missourians Information System) data information on client.
  • Manage oversight of client daily living - i.e. home living routine, cleaning of unit, employment, budgeting, saving, medical care monitoring with Housing Specialist.
  • Review policies and procedures regularly and support the mission of The Salvation Army and Veteran Residence program by treating all persons with respect and dignity, and without discrimination of any kind.
  • Perform duties as assigned (e.g. in-service training, fund-raising/special events; man an information table at community event, etc.).
  • Maintain confidentiality, proper client boundaries, and comply with The Salvation Army Safe from Harm guidelines.
  • Maintain and support a positive work atmosphere by behaving and communicating in a manner that fosters good relationships with co-workers, clients, and supervisors.
Qualifications Bachelor's degree in human service/social service field or related area plus one years of work experience is required. Experience working with recovering substance abusers and veterans is a plus. Combination of education and work experience will be considered. Certified Alcohol and Drug Counselor (CADC) or Recognized Associate Substance Abuse Counselor (RASAC) or LPC or LCSW is a plus. Requires some computer word processing, spreadsheet, and email experience. Excellent interpersonal and communication skills; understanding of the homeless and chemically dependent veteran population. Must be a team player, a self-starter, able to handle several projects at the same time and perform well with minimum supervision. Must demonstrate ability to think independently and exercise sound judgment.
