We have one opening for a Full-Time Transitional Care Coordinator!The Behavioral Health Network is dedicated to helping adults and families struggling with a mental health diagnosis and/or substance use disorders. We are an interdisciplinary team of psychiatrists, nurse care managers, clinicians, care coordinators, and peer support specialists who provide an integrated and mission-guided approach to client care for adults and families.Definition: The Transitional Care Coordinator for the Behavioral Health Network shall be a trained, supervised professional who will provide Care Coordination Services within the Client Access Response and Engagement (C.A.R.E.S.) Team program. As a member of the C.A.R.E.S. team, the BHN Transitional Care Coordinator will work in a collaborative capacity bridging supports to individuals enrolled in all BHN programs while supporting a caseload of individuals requiring assessment, stabilization support, or who may be experiencing service support and life transitions. The BHN Transitional Care Coordinator will provide direct support to requests for services, pre-intake and orientation, access to services, conducting outreach as well as provisioning services to individuals on their assigned caseload.
