Transportation Driver
JOB SUMMARY
Under the supervision of the Transportation Coordinator, is responsible for the direct transportation of physically limited passengers to and from their destinations. Position requires constant interaction with caregivers and community agencies.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Effectively communicates with participants, families, staff and other agencies and departments.Drives a specially equipped van to deliver physically limited persons to varied destinations. Assures that wheelchair lifts and tie down equipment are properly operated and that participants are delivered to appointments in a timely manner.Follows established schedules, adjusts schedules to meet unanticipated changes, determines the safest and most efficient pick-up and delivery routes. Assists passengers on and off the van in a safe and courteous manner. Deals with medical equipment and other emergencies that may arise. Inspects van and maintains it in a clean, orderly and safe condition. Reports the need for maintenance.Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning. Communicates any changes in participant condition to team members. Observe each participant for any change in physical, mental, emotional, and social functioning and shall report such changes to the licensed nurse.Completes documentation as required.Coordination of 24-hour care delivery.
REQUIREMENTS
High school diploma, or equivalent.Minimum 2 years supervisory experience.Minimum 1 year experience transporting individuals with special mobility needs.1 year of experience working with a frail or elderly population preferred. If this is not present, training on working with a frail or elderly population will be provided upon hiring.Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.Upon employment, defensive driving course, first aid, and CPR certifications are required to be kept up to date.BoldAge PACE provides equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
