- Must be completed within 30 days of start.
- American Heart Association certification required for all direct patient care personnel.
- Must be dated within 30 days of start for any position requiring a license or certification.
- All engaged contractors must have signed the Trinity Code of Conduct Form on file.
- Standard i9 completed by the agency.
- Confidentiality of Patient and Proprietary Information and Information System Security Agreement must be signed.
- Respirator Evaluation and Skills Checklist must be completed within 30 days of start.
- Proof of vaccine, declination or exemption required for COVID-19 Vaccine, MMR, Hepatitis B, Influenza Vaccine, Varicella, Tdap.
- Physical/Health Statement from the last year required.
- 9 Panel Drug Screen must be completed and resulted before start.
- Resume, Skills Checklist within a year, 2 Professional References required for Trinity Profile.
- Conflict Of Interest Disclosure Form must be signed.
- Criminal Background Check, Education Verification, EPLS, GSA/OIG, NSO, and System Access Form are required.
