Weekly Pay Range - $1644 To $1827**Job Title:** Licensed Practical Nurse (LPN)**Job Specialty:** Home Health**Job Duration:** 13 weeks**Shift:** Day 5x8-Hour (8:00 AM - 5:00 PM)**Guaranteed Hours:** Not guaranteed**Experience:** Minimum 1 full year Adult Home Health or Hospice experience**License:** Valid and in-hand license required**Certifications:** CPR, BLS**Must-Have:**- Proficiency in HomeCare HomeBase (HCHB) used within the last 6 months- Reliable transportation with a current driver's license and required liability insurance- Ability to travel up to 75 miles from home clinic- Willingness to participate in weekend rotations as needed**Job Description:**- Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies- Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses- Conduct home visits and attend periodic office meetings- Manage a caseload of 5-10 patients per day, equating to approximately 6-8 points- Handle types of cases such as Start of Care, Wound Care, and Follow-up Visits- Attend to different territories as needed, including West, Central, and East- Ensure charting proficiency and complete documentation within set productivity expectations- Participate in orientation and comply with all requirements- Maintain productivity expectations with a minimum of 32 points weekly*Note: The flu vaccine requirement is currently unknown, and COVID-19 vaccine is not required.*
