- Certification Details
- BLS - Basic Life Support
- ACLS - Advanced Cardiac Life Support
- Job Details
- Must have MA license in hand for acceptance
- 1-year plus experience in Case Management, LTAC/Rehab
- CM experience must be specific for hospital discharge planning (rehabilitation hospital experience a plus but not required)
- Familiar with EMR and/or documentation expectations for role (ACMA SOP)
- Familiar with Continuity of Care / Transition of Care requirements (CMS standards)
- Job Requirements
- MA license required
- 1+ years of Case Management experience
- Experience in hospital discharge planning preferred
- Familiarity with EMR and documentation expectations
- Knowledge of Continuity of Care / Transition of Care requirements
- Schedule Information
- Shift: Mon-Friday 8-5p
- Shift is 5x8 days but any variation to 8 hour shift (staying late/coming in early) has to be discussed and approved by manager
- Unit Specific Information
- Unit specializes in rehabilitation and case management
- Additional Details
- Government issued photo ID required
- MA CORI Statewide Release – All travelers must complete ASAP
- TB Questionnaire required for all travelers
