Job Title: RN Job Specialty: Case Management Job Duration: 13 weeks Shift: Monday-Friday, 8:00 AM - 5:00 PM Guaranteed Hours: Not specified Experience: Minimum of 1 year required License: Registered Nurse (RN) Certifications: Not specified Must-Have: - Meditech or HCA experience - Bedside case management experience - Post-acute care knowledge Job Description: - Provide bedside case management to patients, ensuring efficient discharge planning and care progression. - Conduct hard conversations with families, patients, and open discussions with the medical team for effective care planning. - Utilize Meditech/Midas or similar systems for hospital records and patient management. - Work in a team environment with approximately 15 RNs and 15 SWs, handling patient ratios of 1:30. - Display a strong personality with adaptable and proactive qualities. - Adhere to dress code: Navy scrubs with a white lab coat, business casual attire with closed-toe shoes, no crocs, and no fake nails. - Engage in discharge planning and throughput experience. - Participate in a unit with a modern approach, offering opportunities to work with new equipment and technology. Please note that weekend work is required on a rotating basis. There is no on-call requirement for this position.
