**Job Title:** Level 2 Registered Nurse Case Manager - Home Health Care**Job Specialty:** Case Management**Job Duration:** 13 weeks**Shift:** Monday to Friday, Day shift, 8-hour shifts from 8:00 am to 4:30 pm**Guaranteed Hours:** Not specified**Experience:** - Minimum one year of experience as a clinical nurse in a hospital - Nine months of experience in home care preferred**License:** NY License ONLY**Certifications:** - BLS (any provider accepted) - ACLS (AHA ONLY)**Must-Have:** - Certified Home Health Care Experience - OASIS Experience - Experience with EPIC or Netsmart (Preferred) - Current NYS driver's license**Job Description:** - Develop and revise patient plans of treatment and discharge plans in coordination with patients, families, physicians, care teams, and third-party payors- Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and address significant changes in condition within agency-defined time frames- Process medical orders and notify physicians of patient needs and changes in condition; complete certification/recertification orders and discharge summaries- Provide a respectful environment ensuring patient privacy, confidentiality, and property protection- Initiate appropriate preventive and rehabilitative strategies requiring substantial specialized nursing skills- Consult with and educate patients, families, and teams about disease processes, self-care techniques, and preventive strategies- Supervise LPNs to ensure patient health, safety, and compliance with care plans- Direct, coordinate, evaluate, and supervise quality patient care services provided by the home health agency- Interact positively with patients, physicians, referral sources, and others to maintain positive relationships- Support care management philosophy for collaborative, customer-focused planning to meet individual health and service needs for quality and cost-effective outcomes- Assure completion of clinical records required for compliance with state and federal laws, ensuring seamless service delivery within established timeframes- Maintain current knowledge of community-based services- Demonstrate flexibility and willingness to visit patients based on need, not geography- Resolve problems at the point of contact- Ensure patients or responsible parties sign authorization and release of information forms- Attend patient care conferences for assigned patients- Monitor results and progress on quality indicators, including customer satisfaction with RN function- Reduce or eliminate avoidable costs and errors, and respond to Quality Management reviews/audits- Participate in performance improvement and peer review as assigned- Maintain productivity and ensure appropriate visit authorizations and documentation of medical necessity for home care visits- Promote workforce diversity and participate in educational and professional development activities- Demonstrate commitment to professional growth and responsibility for Key Performance IndicatorsEnd of job description.
