We are seeking a skilled Registered Nurse to provide high-quality care in Case Management at BronxCare Hospital Center, Grand Concourse Campus. This role involves working on Med-Surg and Telemetry floors, assisting with discharge planning, and collaborating with social workers and an interdisciplinary team.

• Education: BSN (Bachelor of Science in Nursing) required
• Experience: 1+ year inpatient case management experience (preferred: critical care experience)
• Certifications:
  • BLS (Basic Life Support)
  • PRI (Patient Review Instrument) Certification (can be completed online via the School of Albany)
  • SCREEN Training Certification
