We are actively seeking a dedicated Registered Nurse for a Home Health travel position. This is an exciting opportunity for experienced RNs looking to make a difference in patients' lives through in-home care.

Position Details:

• Start Date: ASAP
• Available Shifts: 8 D
• Pay Rate: $2,349.60 - $2,589.35 per week

Assignment Summary:

• Standard contract assignment: 13 weeks
• Schedule: Monday - Friday, 8am - 5pm
• Weekend rotation: Yes (two weekend days/month, two weekdays/month)
• Region: Home Health
• Territory: Niagara County

This request has been initiated due to increased workload and the need to fill an open Full-Time RN requisition.

Travel Expectations:

• Average mileage per week: 200

Productivity Expectations:

Weekly units: 30 points

EMR System:

Utilize HCHB for documentation.

Qualifications:

• Required: Home Health experience, OASIS knowledge, NY License
• Preferred: Experience with HomeCare HomeBase & OASIS
