Job Title: Registered Nurse (RN) Job Specialty: Home Health Job Duration: 260 days Shift: Day - 5x8 - 9am to 5pm (40 hours per week) Guaranteed Hours: Not specified Experience: Minimum two years of experience in a home setting within the last ten years. Recent experience conducting comprehensive patient assessments is required. License: Current RN Licensure in New York State Certifications: Basic Life Support (BLS) required before the start date Must-Have: - Two years of experience in a home setting within the last ten years, including work with Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. - Experience conducting assessments similar to UAS, such as OASIS. - Providing primary care to homebound persons. - Care management for home and community-based services, including Hospice and Assisted Living. - Home care experience as a paraprofessional or Licensed Practical Nurse caring for a minimum of two patients per week. Job Description: - Perform initial in-home pre-evaluation of patients, including assessment of health status, strengths, care needs, and preferences. - Guide the development of individualized long-term care service plans. - Review consumer medical documentation and health referral forms, as relevant. - Enter evaluation data into electronic forms and transmit as required. - Document any concerns, conflicting information, or issues that arise during the evaluation process. - Emphasize continuity of care, reducing fragmentation, duplication, and gaps in the treatment plan. - Discuss health care options, supports needed, service vendor options, and waiver options with consumers. - Initiate communication between the Call Center and eligible consumers for enrollment in a managed care health plan. - Enhance communication and collaborative relationships with interdisciplinary care team members to improve care coordination. - Collect quality review data and required documentation to support outcome measurements; record case notes into CRM. - Identify opportunities for health promotion and illness prevention. - Maintain a comprehensive working knowledge of community resources, payor requirements, and network services for the target population. - Meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template. - Perform other duties as assigned by the Regional Nurse Manager or project management. Candidates should be prepared to conduct hybrid tele and in-field assessments and sign a conflict of interest form before starting the role.
