Job Title: Registered Nurse - Hospice RN Job Specialty: Hospice Care Job Duration: 14 weeks Shift: 8:00 AM - 5:00 PM, 5x8 Hour Shifts Guaranteed Hours: 40 hours per week Experience: Minimum of one year as an RN in acute care, oncology, long-term care, hospice, or home health License: Current license to practice nursing in Indiana Certifications: BLS (American Heart Association), Current automobile insurance, and valid driver's license Must-Have: - Knowledge of terminally ill patients and hospice care - Excellent patient assessment skills - Understanding of hospice patient and family needs - Experience with managed care, Medicare, and Medicaid - Proficiency in nursing assessment tools, computers, and documentation Job Description: - Provide direct patient care services to hospice patients - Manage an interdisciplinary group including Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff - Assure high-quality care in accordance with the Plan of Care - Perform initial and ongoing patient assessments - Develop and implement the Hospice Plan of Care in collaboration with physicians and the interdisciplinary team - Deliver all-inclusive physical, psychosocial, educational, emotional, and spiritual services - Report changes in patient conditions to attending and hospice physicians - Maintain accurate medical records, including interdisciplinary notes and assessments - Practice infection control measures and standard precautions - Monitor staff scheduling - Participate in the Quality Assessment and Performance Improvement Program - Engage in on-call duties as needed - Promote company core values - Complete required compliance training - Participate in staff meetings - Maintain knowledge about the management of pain and symptoms in the dying process - Conduct routine supervisory visits with team members - Follow up on patient records to ensure standards of practice are met - Communicate with the patient's family for support and continuity of care - Manage education for patients, families, and caregivers regarding the Plan of Care
