Job Title: Hospice Registered Nurse (RN) Job Specialty: Hospice Care Job Duration: 12 Months Shift: Monday to Friday, 8:00 AM - 5:00 PM (5x8 Hr) Guaranteed Hours: 40 Hours per Week Experience: Minimum of 1 year as an RN in acute care hospital, oncology, long-term care, hospice, or home health. School internships and clinical hours may be considered as experience in individual cases. License: Current license to practice nursing in the state of employment. Certifications: BLS (American Heart Association) Must-Have: - Registered Nurse credential - Valid driver's license and current automobile insurance Job Description: - Provide direct patient care services to hospice patients and supportive care to patients' families. - Manage an interdisciplinary group including Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance hospice care quality. - Case manage all assigned patient clinical records ensuring highest care quality in accordance with the Plan of Care. - Perform initial and ongoing assessment of patients. - Help develop and implement the Hospice Plan of Care as prescribed by the physician, collaborating with the Interdisciplinary Group. - Provide comprehensive physical, psychosocial, educational, emotional, and spiritual services including bereavement support to patients and families. - Report changes in patient conditions to both the Attending and Hospice Physicians. - Maintain accurate and updated medical records inclusive of all interdisciplinary notes and plans of care. - Practice infection control measures and standard precautions. - Monitor scheduling of staff. - Participate in Quality Assessment and Performance Improvement Program. - Participate in the agency on-call schedule as requested. - Promote company core values consistently. - Complete compliance annual training. - Attend staff meetings regularly. - Maintain high knowledge level in pain and symptom management in the dying process. - Make routine supervisory visits to ensure quality services and compliance. - Maintain primary control and professional management of each patient, acting as liaison among the physician, patient/family, and hospice team. - Educate patients, families, or caregivers in relation to needs identified on the Plan of Care. - May pronounce death in the absence of an attending physician. - Supervise aides and conduct an initial competency evaluation for all tasks assigned to aides. - Prepare written instructions for aide-provided care, consistent with the interdisciplinary plan of care. - Make and document supervisory visits every two weeks to assess aide performance, ensuring competency in all job tasks.
