- Proof of I9 or Attestation
- Government issued photo ID required. Photo Identification must be carried on person during hours of service to client.
- Proof of Everify or Attestation
- Please require if nurse is working on a compact license
- Flu Vaccine required during flu season (Oct.1 - March 31). Facility specific exemption request are due 11/1, or 15 days from hire date. Exemptions are on a case by case basis. If exemption approved candidate must wear mask.
