- Certification Details
- COVID-19 Vaccine - Preferred, not required.
- MMR - Must show a new positive titer after booster series.
- Fit Test - Completed within 1 year of start updated annually.
- Hepatitis B - Proof required or signed declination.
- Positive PPD- Negative Chest X-Ray and Annual TB Questionnaire if positive.
- Tdap - Proof of vaccination within past 10 years.
- Job Details
- Replacement for ESS leaving Ahuja.
- Must have ACLS/BLS
- Must have min. 2 years of experience
- Job Requirements
- BLS - American Heart Association only. Required for clinical positions.
- ACLS - American Heart Association only.
- License/Certificate Verification - Primary Source Verification required. Temporary Licenses are NOT accepted.
- UHS Skills Checklist - Skill specific required to be complete within 1 year prior to start.
- UHS Competency Exam - Competency Exams for the relevant department must be complete within 1 year prior to start.
- UHS Core Competencies - To be complete within 1 year prior to start.
- Physical/Health Statement - Physical Exam completed within one year of initial start date.
- UHS Criminal Background Check Results - To be run upon initial hire.
- UHS OIG Search - To be complete within 6 months prior to start.
- UHS SAM/EPLS/GSA/OFAC - Supporting documentation required.
- UHS Drug Screen - To be run upon initial hire.
- Additional Details
- American Heart Association only. Required for clinical positions.
- Primary Source Verification of professional license and/or certification, as required for the position ran within 30 days of start updated upon expiration.
- Proof of vaccine series or completion and approval of client medical or religious exemption form.
