Job Description: - Work a minimum of 4 weekend shifts in a 6-week period. - Participate in the development of treatment plans and protective services process. - Ability to organize and participate in family meetings. - Experience with HMOs, IPAs, etc. - Competence in arranging DME, SNF, and ambulance transport independently. - Must commit to working 3 out of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day). - Expected to float between all facilities within a given state as needed. - Participate in required training and orientation modules pre-start and annually. - Collaborative engagement with patients with mental health history or in crisis, patients experiencing homelessness, and those needing drug/alcohol abuse treatment options. - Engage with the processes related to advanced directives and POLST. - Basic knowledge of adoption/foster care practices.
