TravelNurseSource is working with Cynet Health to find a qualified Case Manager RN in Apple Valley, California, 92307 Pay Information $2,430 per week About The Position Job Title: Registered Nurse Job Specialty: Case Manager Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 16:30) Guaranteed Hours: To be discussed Experience: To be discussed License: Valid RN License Certifications: To be discussed Must-Have: Minimum of 2 supervisory references covering 1 year of employment total within the last three years in a comparable specialty/setting. Job Description: - Work a minimum of 4 weekend shifts in a 6-week period. - Participate in the development of treatment plans and protective services process. - Ability to organize and participate in family meetings. - Experience with HMOs, IPAs, etc. - Competence in arranging DME, SNF, and ambulance transport independently. - Must commit to working 3 out of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day). - Expected to float between all facilities within a given state as needed. - Participate in required training and orientation modules pre-start and annually. - Collaborative engagement with patients with mental health history or in crisis, patients experiencing homelessness, and those needing drug/alcohol abuse treatment options. - Engage with the processes related to advanced directives and POLST. - Basic knowledge of adoption/foster care practices.
