- Job Details
- LCSW Social Worker masters prepared with Maryland license prior to interview.
- Must have 3 -5 years of recent acute discharge planning experience in the inpatient hospital setting.
- 2 positions are available.
- The social workers will be assigned to various units primarily; oncology, med/surg, and ICU.
- Will work every other weekend.
- Required to do assessments and discharge planning plans for patients requiring all types of needs varying from nursing home placements, home health services, dealing with homelessness, crisis patients, uninsured patients, etc.
- Rarely could be asked to cover shifts at sister hospital located in Germantown.
- Job Requirements
- Maryland license prior to interview.

- Schedule Information
- Will work every other weekend.
- Unit Specific Information
- Assigned to various units primarily; oncology, med/surg, and ICU.
- Additional Details
- All engaged contractors must have page 27 signed form on file in Trinity VMS.
- Must be completed within 30 days of start.
- American Heart Association certification is required for all direct patient care personnel.
- Proof of vaccine Sept 2024 booster, declination or exemption(with agency letterhead).
- Declination not accepted for CA or IL.
