Job Type Full-timeDescriptionPosition Title: Advanced Practice Provider (Nurse Practitioner, Physician Assistant)Department: Valor Health Family Medicine and Urgent CareSupervisor's Title: Executive Director of Physician Services and Clinic AdministratorI Position Summary:The purpose of the Advanced Practice Provider is to provide continuity of patient care. This is a broad spectrum rural family practice that serves all ages and conditions. The Advanced Practice Provider must exercise sound professional judgment while providing urgent care and primary health care within the scope of their practice to the patients served by Valor Health. Urgent care and primary care services shall include preventive, corrective, and promotive care to patients from infancy to old age including early stages of illness to long term disease conditions as well as emergency situations. II. Principal Functions and Responsibilities:A. Evaluate the physical and psychosocial health status through a comprehensive health history and physical examination.B. Diagnose, treat, and help prevent diseases and injuries during normal office hours that commonly occur in the general population with patients in the clinic with consultation from supervising physician as needed and in accordance with the Idaho State Board of Medicine and Board of Nursing Guidelines. C. Accurately and timely document for medical necessity, as defined by Medical Staff Rules, patient information including medical history, clinical condition, medical interventions utilized for treatment, and plan of care. D. Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury.E. Incorporate preventive health care provisions aimed at improving and maintaining health status based upon individual, cultural, sociological, psychological and environmental factors.F. Write and order diagnostic studies for laboratory, x-ray, electrocardiograms, and other special tests as allowed in the Delegation of Services. G. Provide guidance and applicable delegation to clinical staff including, but not limited, to RN's, LPN's, MA's, CNA's, Diagnostic Imaging Techs, and/or Laboratory Techs/Scientists.H. Interpret reported information in accordance with knowledge of the laboratory or diagnostic studies, provided such laboratory or diagnostic studies are related to and consistent with the practitioner's scope of practice and experience. I. Effectively conduct procedures and services that are within the scope of service and in accordance with Valor Health and clinic policies.J. Promote patient centered model of care within scope of competence and exercise judgment when case should be referred to a physician, the physician of the patient's choice, or to available specialists for any condition which in the judgment of the Advanced Practice Provider requires expertise beyond which he/she is able to offer.K. Advise patients and community concerning diet, activity, hygiene, and disease prevention.L. Maintain confidentiality in matters relating to patient and family.M. Effectively interact with patients/families with a variety of developmental and socio-cultural backgrounds.N. Maintain a safe environment by recognizing, eliminating and/or reporting hazards.O. Actively participate in Risk, Compliance, Quality, Performance Improvement, Patient Safety, and Infection Prevention programs as deemed necessary by the organization.P. Participate in the development, execution and periodic review of the written policies governing the services of the clinic.Q. Complete all necessary paperwork, including documentation to support claim submittal, records requiring provider certification, and sign off on dictation in a timely manner.R. Assure that adequate patient health records are maintained and transferred as required and in a timely manner.S. Follow Organization Policy, Medical Staff Bylaws and Rules.T. Other duties as assigned.RequirementsIII. Position Qualifications/Requirement:A. Current, unrestricted Physician Assistant licensure in Idaho or current, unrestricted Advanced Practice Professional Nursing licensure in Idaho.B. Current Basic Life Support (BLS) Certification Card.C. Current DEA certificate without restriction.D. PALS or NRP, ACLS, as required.C. Ability and willingness to demonstrate and maintain competency as required for job title and the unit/area(s) of assignment.D. Excellent communication skills. E. Ability and willingness to work with the particular growth and development needs of the patients in the area of assignment.F. Ability to manage a complex work environment related to changing customer needs, including work with patients with acute, chronic and complex disease processes.G. Ability and willingness to exhibit behaviors consistent with high standards for performance improvement and organizational values.H. Ability and willingness to exhibit behaviors consistent with principles for service excellence.I. Knowledge about professional scope of practice.J. Ability and willingness to work within professional scope of practice.K. Ability and willingness to work as part of a team-based model for care delivery.
