This employee functions under the direction, instruction and supervision of the Registered Nurse (RN). Care for the patient is given where the patient resides i.e.: Patients home or FacilityQUALIFICATIONS
- Current Certified Nurse Aide (CNA) in the state of Pennsylvania preferred
- Minimum of two [2] years experience in patient care preferred
- Ability to understand and carry out directions and instructions and maintain appropriate records
- Current CPR or First Aid certification
- Excellent organizational skills with attention to detail
- Outstanding communication skills (both written and verbal)
- Reliable independent means of transportation with a valid Pennsylvania driver's license and at least the minimum state required vehicle insurance
RESPONSIBILITIES1. Provides personal care according to the established individualized plan of care as developed by the RN member of the team such as (but not limited to): bathing, grooming, dressing, shaving, feeding; checking vital signs; assisting with ambulation; changing position in bed; assisting with transfers, changing bed linens; assisting with elimination; providing oral hygiene, skin care, foot care, ear care, light house cleaning and meal preparation2. Coordinates and communicates a weekly patient schedule with the RN Case Manager and Licensed Practical Nurse (LPN) to meet the optimal needs of the patient and family3. Completes all computer and/or paper documentation requirements within the timeframes specified4. Participates in patient and/or family teaching activities as directed by the RN team member5. Together with the team, provides care as established by the IDG to meet the physical, emotional and spiritual needs of the patient as necessary, and appropriately supports the family at the time of death6. Maintains regular communication and interaction with patients, families, physicians and hospice team members7. Participates in required on-call responsibilities8. Attends and participates in staff meetings and other related activities when required9. Completes required in-services; ensures all required continuing education credits are obtained for certification renewal if applicable10. Demonstrates an understanding of the Quality Assessment Performance Improvement (QAPI) process, hospice policies and procedures, and participates in projects when requested11. Participates in the orientation of new staff members when requested12. Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards, and Concordia Hospice of Washington policies and procedures; maintains confidentiality of patient and employee records and information; has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to the Executive Director and/or Compliance Managerjob Types: Full-timeBenefits:
