Job PurposeThe Veteran Coordinator (VC) at FCC is responsible for ensuring that Armed Forces members and veterans receive comprehensive behavioral health services in alignment with the VHA's clinical mental health guidelines. This role involves coordinating care, facilitating access to services, and maintaining regular communication with veterans and relevant stakeholders.Qualifications
  • A master's or bachelor's degree in a human services field from a college or university included in the US. Department of Education's database of accredited schools at http:/ope.ed.gov/accreditation, OR.
  • Qualified Addiction Professional (QAP), OR
  • Associate of Applied Science degree in Behavioral Health Support as designated by the Department of Mental Health.
  • Bachelor's degree unrelated to human services field with two years of qualifying experience; OR
  • Four years of qualifying experience
    • Qualifying Experience must include delivering services to individuals with mental illness, substance use disorders or developmental disabilities. Experience must include at least one of the following:
      • Providing one-on-one or group services with a rehabilitation/ habilitation and recovery/resiliency focus;
      • Teaching and modeling for individuals how to cope and manage psychiatric, developmental, or substance use issues while encouraging the use of natural resources/ supports;
      • Supporting individuals in their efforts to find and maintain housing, employment, and/or function appropriately in a family, school, and community;
      • Assisting individuals to achieve the goals and objectives on their individualized treatment plan.
  • Experience in behavioral health services, particularly with Veteran and military personnel. Veteran status preferred, but not limited to.
  • Knowledge of Veteran Health Administration (VHA) clinical mental health guidelines and TRICARE programs.
  • Strong coordination and communication skills.
  • Ability to work collaboratively with internal and external stakeholders.
Core Job Duties
  • Work with FCC clinical programs to identify any Armed Forces members or veterans who may need assistance.
  • For those on the VC's caseload:
    • Assist and encourage communication with the primary care manager for referrals outside the military treatment facility (VAMC), with the client's consent.
    • Aid Active-Duty Service and Active Guard/Reserve Members residing more than 50 miles from a military clinic with TRICARE Prime Remote enrollment.
    • Help schedule appointments with TRICARE-authorized providers for members eligible for TRICARE Reserve Select, upon client request.
    • Assist veterans in enrolling in VHA for health and behavioral health services.
    • Maintain regular contact with veterans, as clinically indicated.
    • Coordinate care for veterans with behavioral health conditions, substance use disorders, and co-occurring conditions by collaborating with the FCC treatment team and external agencies.
  • Track the number and type of services provided to veterans.
  • Participate in community events that support and engage with veterans.
  • Adherence to all applicable evidence-based practice models.
  • Orally communicate information effectively and accurately.
  • Assure program operates within the constraints of the agency's certification (CARF, DMH) standards and strategic plan related to clinical care.
  • Abide by program and agency policies and procedures.
  • Other job duties and special projects as assigned.
