If you believe in the empowerment of women, children, veterans, and their families, please consider the employment opportunity below.The Village Advocate provides ongoing support services and expertise through comprehensive assessment, planning, implementation, and referral, to address the needs of participants by identifying necessary wrap around services to support their treatment and increase knowledge and skills. Responsible for identifying and coordination of services across various agencies involved with the participant to include DFCS, Family Treatment Court, Probation and Parole. The overall goal of the position is to enhance the quality of care and to promote continuity of services both during and after treatment.ESSENTIAL JOB RESPONSIBILITIES
- Ensures that new intakes are greeted, orientated to case management services, and immediate needs are identified and addressed within 48 hours.
- Secures information such as medical, psychological, and social factors contributing to participant's situation, and evaluates participant's capacities and service needs.
- Aids with obtaining identification and vital records as needed.
- Interview participants to establish goals and objectives and develop individual service plans.
- Meets with participants weekly to discuss progress and/or barriers to meeting identified goals and objectives.
- Advises participant individually, in family, or in other small groups regarding plans for meeting needs, and aids participant to mobilize inner capacities and environmental resources to improve social functioning.
- Completes career development assessment.
- Responsible for obtaining participant's DFCS/FTC case plan and coordinating with Family and Children Services Director to establish objectives to meet goals, as needed.
- Reviews individual service plan and performs follow-up to determine quantity and quality of service provided participant and status of participant's case.
- Assesses and records participant and community resource information.
- Establishes links and builds relationships with all agencies and service providers.
- Makes referrals to service providers for participants based upon needs and requests.
- Monitors DFACS Visits, Probation visits, and Parole visits.
- Facilitates process with Drug Court, ensuring compliance of participant, going to court and reporting progress of participant.
- Provide linkage between services compliance with state, federal and accreditation and regulatory agencies.
- Assists in coordination of mental health and medical services and follow-up with Program Service Counselor and Medical Department.
- Transporting participants to appointments and meetings as necessary.
MINIMUM QUALIFICATIONS
- Bachelor's degree (B. A.) from four-year college or university; or three to four years related experience and/or training; or equivalent combination of education and experience.
- Experience working with diverse populations.
- CARES, CPS, CADC-T, Case Management certifications, preferred.

- Basic computer skills and knowledgeable of Microsoft Office Products.
