We are seeking a compassionate and dedicated Weekend Visit Nurse to provide high-quality healthcare services to patients in their homes or community settings. The ideal candidate will possess a strong background in nursing with expertise in various medical disciplines, including internal medicine and behavioral health. This role requires excellent communication skills, the ability to work independently, and a commitment to delivering patient-centered care. Hudson Valley Hospice weekend scheduled visits nurse provides nursing care after office hours to Hudson Valley Hospice patients and their families. The nurse ensures that services are consistent with the philosophy, goals, objectives, and policies of the organization. We are seeking an experienced RN who will commit to a weekend schedule (Friday, Saturday and Sunday) at 32 hours per week. This position will come with Full Time Benefits and Pay!ESSENTIAL FUNCTIONS
- Performs pre-scheduled visits to transitioning, actively dying, or complex newly admitted patients.
- Display flexibility to perform emergent on-call visits as needed.
- Provides services according to the patient/family care plan in a homecare, inpatient, and/or nursing home setting.
- Make visits to patients at home, in hospitals or nursing homes as needed or directed by the On-Call Coordinator, Director of After-Hours Services, Weekend workflow supervisor, or the executive supervisor on-call.
- Responsible for obtaining reports prior to the start of the on-call shift.
- Provides accurate and complete reports as required at the end of each on-call shift.

WORK ENVIRONMENT/PHYSICAL DEMANDS The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
- Possible exposure to highly contaminated agents.
- Moderate to high risk of exposure to unsafe environments.
- Routine duties involve the potential for direct exposure to blood or body fluids.
- Regular travel by car in all weather conditions.
- Works in patient homes and, on rare occasions, in hospitals and nursing homes.
- Ability to carry up to twenty-five pounds of equipment and/or supplies.
PREFERRED EDUCATION & EXPERIENCE
- Graduate of an accredited nursing school.
- Valid New York State RN license
- Three years of nursing experience including one year of medical-surgical experience preferred.
- Experience in Hospice, community health, and/or oncology nursing preferred.
- Valid New York State driver's license with reliable transportation
- Hospice and Palliative Care Certification or after completion of one year of employment is highly encouraged
