Wellness Coach (Brooklyn) - Tandym Group : Job Details

A social services organization in NYC seeks a Wellness Coach to support high-risk populations in a 45-bed co-ed facility.About the Opportunity:

• Location: Brooklyn, NY (Onsite)
• Schedule: Saturday to Wednesday
• Hours: 2pm to 10pm

Responsibilities:

• Provide case management and develop individual support plans
• Assist with daily living skills and crisis intervention
• Accompany residents to medical/psychiatric appointments
• Facilitate community integration activities
• Perform other duties, as needed

Qualifications:

• 2+ years of experience working with the Behavioral Health populations

Desired Skills:

• Bachelor's Degree
• Bilingual (English and Spanish)
• Experience with mental illness, substance use, and homelessness
