Open Door Family Medical Center provides health care and wellness programs to individuals and families in need throughout Westchester, Putnam and Ulster Counties. A pioneer Federally Qualified Health Center, Open Door provides more than 300,000 patient visits annually and serves nearly 60,000 individual patients who might not otherwise have access. Open Door offers integrated services and a holistic approach to building healthier communities. Primary medical care, dental care, integrated behavioral health care, clinical nutrition, wellness programs, and chronic disease management are the foundation of its clinical programs.

Founded as a free clinic in 1972, Open Door's mission of building healthier communities through accessible, equitable, culturally competent health care has led to site and service expansions. Today, Open Door operates centers in Brewster, Mamaroneck, Mount Kisco, Ossining, Port Chester, and Sleepy Hollow, in addition to eight School-Based Health Centers in the Port Chester and Ossining School Districts, a mobile dental van in Mount Kisco, and a new dental practice in Saugerties, NY.

Open Door is seeking an experienced (Nurse Practitioner or Physician Assistant) to work in our Port Chester NY site. This role will report to the Medical Site Director.

RESPONSIBILITIES:

  • Provide high quality primary care to our female patients.
  • Provide comprehensive primary, preventive, and chronic disease care to ensure that the needs of patients are met on an ongoing basis.
  • Ensure that high quality care is delivered cost-effectively.
  • Provide appropriate referrals to specialists or hospitals for further evaluation and management, when indicated.
  • Maintain electronic medical records in compliance with Open Door's standards with high efficiency.
  • Provide outpatient continuity of care for patients discharged from hospitals.
  • Engage patient, family and cross-functional care teams in sustained health and well-being of the patient.
  • Partner with Open Door's Behavioral Health team and resources.
  • Maintain familiarity with ongoing research and development in relevant fields.
  • Meet or exceed performance improvement initiatives, supported with timely and accurate data.
  • Represent Open Door in community outreach activities.

REQUIREMENTS:

  • Degree and licensure to practice in New York as a Family Nurse Practitioner or Physician Assistant.
  • Proficiency in Spanish, preferred or Medical Spanish knowledge.
  • A minimum of one year relevant experience. Community-based, out-patient experience is highly desirable.
  • Outstanding “bedside manners”: patient-centric with empathy, excellent active listening and communication skills.
  • Outstanding clinical capabilities and judgment.
  • Passionate about providing high quality primary health care to low-income, under-served residents and those lacking access to health care.
  • Focused on collaborative team-based care.
  • Responsive and capable of working in a high accountability environment.
  • Comfort level with using an electronic health record system.
  • Flexible.
  • Excellent judgment and ability to prioritize resources where they will be most useful.
  • Comfortable receiving and providing constructive feedback.
  • Engaged in learning and continuous quality improvement.
  • Outstanding written and verbal communications skills with a culturally diverse patient and staff population.
