Qualifications:
Registered in State of New York as a Registered Nurse. Has the ability to render care, report changes in residents condition and coordinates with Nursing, Attending Physician and Wound Care treatment recommended for resident.
Specific Responsibilities:
Wound Nurse
1. Weekly wound rounds with Wound Physician.
2. Responsible in ensuring that treatment recommended by Wound Physician is ordered by Attending MD promptly and carried out by Nursing and entered in the treatment record.
3. Checks treatment record Q weekly to ensure that treatment is in place and signed by nursing
4. Checks admission and readmissions if admitted with pressure ulcer or skin breakdown
5. Updates ULCER/WOUND tracking sheet and Care Plan in each unit Q weekly
6. Submits to DNS weekly every Thursday pressure Ulcer/Wound tracking
7. Responsible in ordering special mattress for resident with pressure ulcer as needed. Keep a tracking record of all ordered special mattress and responsible in ordering mattress
8. Responsible in calling the company if the resident is transferred out to the hospital or when the resident does not need the special mattress for return and pick up the logs in the book
9. Check if resident has orders for preventive and protective skin care/devices and coordinates with the unit nurse and or the unit supervisor.
10. Check with the unit supervisor residents with reported new skin break. Make sure that the nursing supervisor obtains treatment for the resident. Facility acquired pressure ulcer must have a pressure ulcer investigation completed by nursing supervisor.
