Job Type Full-timeDescriptionAs the Chinese Translator-Care Navigator for Maimonides Hospital, the role is to enhance patient care, patient experience, and transitional care of Chinese patients in the Emergency Department, in-patient setting, and outpatient units, as well as the cancer center. The goal is to promote patients' bio-psycho-socio-economic well-being and connection to community resources and support. Furthermore, the Chinese Translator-Care Navigator helps facilitate appropriate follow-up care when patients return to the community in order to reduce patients' readmission risk. Responsibilities
- Assist patients in navigating their care in a medical setting, including simple interpretation for Chinese-speaking patients.
- Expect community referred patients, and greet and assist with registering patients in the hospital setting.
- Foster communication between hospital and referring community physicians.
- Identify and engage with patients that require additional community support and make referrals to the CAIPA Care team in order to facilitate connection to community resources.
- Update or add PCP information to the hospital system in order to ensure that discharge records will be transmitted to the correct PCP following patient discharge.
- Provide psychosocial support to patients and their family members. (only applicable to Cancer Center)
- Organize and facilitate support group programs for Chinese-speaking patients in collaboration with the Cancer Center teams. (only applicable to Cancer Center)
- Liaise with the Asian Outreach department in order to troubleshoot challenging patient issues.
- Facilitate the safe discharge of patients from the ED and Inpatient units by collaborating with case managers, nurses, and doctors.
- Follow up with patients post-discharge in order to promote timely and appropriate follow-up care, enhance medication compliance, and identify issues needing further assistance.
- Assist patients in making specialist appointments through appointment scheduling and the Asian Outreach department.
- Remind patients of upcoming specialist appointments and assist with rescheduling as needed (only applicable to Outpatient)
Requirements
- High School graduate or equivalent
- Bilingual in Chinese required
- 1 years of work experience in Health Care preferred.
- Care Management experience preferred.
- Mobility required
- Good oral and written communication skills

Salary Range: $48k-$62k
