Job Description :

A community services organization in New York City is currently seeking a licensed Registered Nurse (RN) to join their staff in Manhattan as their new Case Manager. In this role, the Case Manager (RN) will be responsible for developing plan of care and coordinating the delivery of healthcare services for persons receiving services in an assigned program unit. About the Opportunity:

  • Schedule: Monday to Friday
  • Hours: 8am to 4pm
  • Patient Population: Adults
Responsibilities:
  • Coordinate the delivery of routine and specialty healthcare services to address assessed needs of persons receiving services in assigned program unit
  • Assess and establish healthcare outcomes for persons receiving services
  • Develops plan of nursing services and oversees the implementation of the health care plan by educating person's receiving services, DSP/Caregivers and families in the plan of nursing services.
  • Educate, monitor and supervise staff in carrying out ordered treatment plan related to health needs
  • Evaluate the effectiveness of preventive/ongoing healthcare and makes recommendations/ referrals for alternative care, as necessary
  • Act as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providers
  • Complete and maintain all required written documentation on the approved electronic health record system
  • Conduct general and individual specific health related trainings for persons receiving services, direct care staff, and other team members, as needed.
  • Complete relevant medical documents, MIPS and other required consent packets, surrogate packets, writing incident reports, death reports, etc,
  • Performs all other duties as assigned.
Qualifications:
  • Graduate of an accredited School of Nursing
  • NYS Registered Nurse (RN) license
  • Solid assessment, clinical, and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
Desired Skills:
  • Bachelor's Degree in Nursing
  • Experience working with persons with Developmental Disabilities
