Diabetes Educator Responsibilities:
- Conduct diagnostic evaluations to assess patient needs and develop individualized education plans
- Provide education and training to patients and their families on various topics related to their condition
- Collaborate with healthcare professionals to ensure coordinated care and effective patient outcomes
- Monitor patient progress and adjust education plans as needed
- Document patient interactions and maintain accurate records
Experience:
- Previous experience in patient education or a related field is preferred
- Knowledge of tube feeding and patient assessment is highly desirable
- Strong communication skills, both verbal and written
- Ability to work independently and as part of a multidisciplinary team
- Excellent organizational skills and attention to detail
Requirements - Certified Diabetes Educator with a minimum of 3-5 years experience
- Bachelor's Degree in Nursing, required.
- Current license to practice as a Registered Professional Nurse in New York State, required.
OR
- Bachelor's degree in food, Nutrition or Dietetics, required.
- Registered Dietitian, required.
