JOB SUMMARY
  • The Quality Improvement RN is part of the management team of the certified home health agency, and assumes responsibility for the management and coordination of the agency QAPI Plan/Risk Management/Infection Control and education programs to ensure compliance with CHAP, Federal and New York State regulatory standards and requirements. Serving as an educator, the Quality Improvement RN evaluates staff educational needs and develops/provides educational programs based on clinical needs and interests. She integrates current home care regulatory and evidence-base best practice knowledge when orienting staff or providing direct care. Responsibilities include analysis of data for trending and process improvement and utilization review. The Quality Improvement RN meets all of the Principle Duties and Responsibilities of the Registered Nurse and has a visit component expectation
RESPONSIBILITIES
    • Oversees the quality assurance performance improvement program for home care to include quality management of the OASIS data & reporting; process & outcome reports; occurrence reports; home health compare & patient satisfaction data; program quality monitoring data; utilization data, payer inquiry requests, denial activity; fall data and infection control practices; development of policies & procedures. Uses data trending to initiate & sustain performance improvement activities. Coordinates findings with clinical and finance leadership
    • Serves as a regulatory resource for Federal, State and CHAP regulations & standards.
    • Develops, monitors, and evaluates the corrective action plans for deficiencies identified through the quality review process and survey process. Coordinates findings with billing & finance as appropriate.
    • Responsible for chart workflow processing/review of documentation for accurate submission of OASIS data to CMS for Start of Care admissions (SOC) , Resumption of Care (ROCS), and Recertification (Recert) information aligned with condition of Participation and Conditions for billing.
    • Responsible for the compiling data reports/data collection for monthly audits to ensure compliance with Agency policies/procedure, CHAP Accreditation, NYS state and federal regulations.
    • Oversees the Fall and Infection control programs to identify and data trend with performance improvement recommendation to reduce adverse event and rehospitalization data reports
    • Evaluates the need for and plans/coordinates special classes, conferences, and meetings to improve level of staff performance and quality of care.
    • Participates in the weekend/holiday clinical supervisor schedule.
    • Participates in WMC MidHudson Regional hospital committees
    • Fulfills all compliance responsibilities related to the position.
    • Develops, conducts and evaluates programs that assist in meeting the educational needs of home care clinical staff competency.
    • Assists in the development, implementation, interpretation, and evaluation of educational training program standards, policies, procedures, and best practices appropriate to learner needs, desired outcomes, content, and context.
    • Utilizes Learning technologies to develop, launch, and monitor instructor-led, in-service, and online learning as assigned.
    • Collaborates with nursing leadership to provide input into the evaluation of staff performance and readiness to assume staff responsibilities post educational initiatives.
    • Can conduct field supervisory visits home visits with clinical staff to ensure a competent workforce.
    • Performs other duties as required.
QUALIFICATIONS/REQUIREMENTS
  • EXPERIENCE
    • A minimum of two years of home care practice setting required
  • EDUCATION
    • Graduate of an accredited school of nursing required
  • LICENSES/CERTIFICATIONS
    • Current NYS RN license required
    • Valid NY State Driver's License
  • OTHER
    • Understands Home Care regulations Conditions of Participation
